PCP exam

 

I established with a PCP through zoomcare - which turned out to be fairly useless. I thought I would get a list of my symptoms in the notes, but alas, they weren't included.  I'm going to request the notes from Dr. Perez in 2018.

To do:

I need to possibly see Dr. Perez again.

Or get a PCP at OHSu.
